Minnesota law requires that every child attending school be immunized against diphtheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B and varicella (hepatitis A and meningococcal beginning September 2014). The chart "Are Your Kids Ready" outlines the immunization requirements. A student must have all immunizations up-to-date, with a record on file in the health office, or the appropriate form signed, prior to starting school.
No student shall be required to receive an immunization which is contrary to a medical reason or conscientiously held belief. If you are conscientiously opposed to immunizations your notarized signature is required on file in our office.
